- Prosperity - Level (standard) health class regardless of when prescribed insulin

- United Home Life - Preferred (premier) health class if you are prescribed metformin (pill), otherwise standard (deluxe) health class
Diabetics WITH Complications

- Transamerica - Level (insulin is standard rating) if last even was more than 2 years ago
- United Home Life - Will accept most complications, a GREAT place to start!
I have highlighted United Home Life (UHL) above because they have the most liberal and "least strict" underwriting requirements for Diabetics. UHL has multiple categories of health classifications, so if you have been declined elsewhere OR you have serious or combinations or complications from diabetes, UHL is a great carrier and policy to research.
